Pyomyositis is a subacute, deep, bacterial infection of muscle and soft tissue and is predominantly seen in tropical countries. The disease is more common in immunocompromised subjects such as those with HIV infection, renal failure, or undergoing chemotherapy. We present a rare case of multifocal pyomyositis with pyopericardium in an immunocompetent young female. Our patient, despite surgical drainage and empirical antibiotic treatment, progressed to the bacteraemic stage with multiple abscesses and pyopericardium. She was given antibiotics and pericardiectomy was done for the management of pyopericardium. The patient made an uneventful recovery. Although pyomyositis is rare in patients who do not have a predisposing condition, it should be suspected, investigated, and aggressively managed.
